skin problems

Last year our JRT that is 9 years old now. Started to have hot spots.. This year last month I took him to the vet for a hot spot shot again- He came back with tons and tons of fleas bit this time his skin never got better!! Under the belly around the butt and under arm areas are very very red skin.. I am starting to believe that he mite be alerject to something ? What do you think it is. Or if I take him to the vet again - What should I have them look for ?? Because it is tight on money at this time..
